Problem summary
Using Duplicate>Duplicate Hierarchy on the Highpoly mesh results in the duplicate mesh being incorrectly duplicated.
This appears to be a scene-specific bug.

Steps to reproduce
1) Download and open the attached file.
2) Select the Highpoly mesh found within the Blockout folder.
3) Right-click this mesh and select Duplicate>Duplicate Hierarchy.
4) Notice that the output mesh has been duplicated incorrectly.
Expected behaviour
The mesh and its operations should be duplicated.
Actual behaviour
The duplicated mesh seems to be frozen as it doesn't have any mesh operations present.
Workaround
Unknown.
