Problem summary
Monitor out using the AJA T-Tap Pro no longer works at 4K resolutions
This affects both Nuke and Nuke Studio
This affects all UHD and 4K DCI Display Modes, and changing the Pixel Format makes no difference

Steps to reproduce
1) Open Nuke Studio on a machine with the AJA T-Tap Pro installed and connected to a display via HDMI
2) Import any 4K footage, and add it to the timeline
3) Open the Viewer Monitor Output panel and open the Monitor Out advanced settings (the cog wheel)
4) Set the Device to TTapPro, and check Enable Monitor Out
5) Change the Display Mode to HD 1920 x 1080 - 25p and the Pixel Format to 8 bit 4:2:2, you should see the viewer output on the connected display
6) Change the Display Mode to UHD 3840 x 2160 - 25p, the display will now show black
Expected behaviour
For the AJA T-Tap Pro to send a 4K image to the display
Actual behaviour
The display shows black, rather then the output image
